Available for customers who wish to pay for services via bank draft

Utility Billing Change of Address/Name Form 
Required for existing customer changing addresses

Request for Additional Utility Services
Required for existing customer adding services to their current utility
Roll-out Cart / Recycling / Dumpster Orders - 1 time or monthly

Utility Disconnection Request
Must be completed by customer (Primary Name on Account) to disconnect services.

Utility Billing Water Purchase Form
Commercial Users required to complete this form 
Only the hydrants at City Hall and N. Roosevelt Street are used for this purpose

Water Service Turn On for 48 Hours
Available for property owners who have previously had an active account.
This service is available for temporary use, i.e. cleaning purposes.
